This is a Colombian percussion instrument called a "guiro," Spanish for scraper. The guiro is popular throughout Latin-American. It is made from a hollow gourd with parallel notches cut in one side.

The musician plays the guiro by holding the gourd in the left hand with the thumb inserted into the back sound hole to keep the instrument in place. The right hand usually holds the scraper, called a "pua," and plays the instrument. Playing the güiro usually requires both long and short sounds, which are made by scraping both up and down in long or short strokes. It is played by rubbing a stick or tines along the notches to produce a ratchet-like sound.
